The problem isn’t your technology. It’s the assumption built into your detection logic: that fraud looks like the fraud you’ve already seen. Every SIU program I’ve worked with has the same gap. The model performs. Cases open and close. Recoveries get made. And somewhere in the portfolio, the same scheme is running quietly, below the threshold, through a gap nobody has named yet. This piece is about that gap, where it lives, and what actually fills it.
Detection Logic Is Built on Historical Fraud
Every fraud detection model is trained on past patterns. The billing anomaly that got caught two years ago. The provider network that triggered flags in the last audit cycle. The referral pattern that matched the one investigated in a prior case. The model learns what fraud looked like and gets better and better at recognizing it again.
This is the structural vulnerability that every SIU program carries. Your detection logic is optimized for the fraud you’ve already seen. The fraud that’s currently running through your portfolio looks different: not categorically different, but different enough. Different billing sequences. Different provider relationships. Different authorization patterns. Different enough to clear the model.
Fraud schemes evolve in direct response to detection. Bad actors learn what gets flagged. They adapt. Your detection logic, built on historical patterns, has a structural lag that sophisticated fraud exploits by design.
The Behavioral Gap: What Models Don’t See
Pattern detection excels at identifying statistical anomalies: billing frequency, claim volume, amount distributions, timing clusters. What it doesn’t catch is behavioral sequence, the specific ordering of actions, documentation, and communications that characterizes a scheme rather than just its output.
Consider a provider committing authorization fraud. The claim amounts are within normal range. The billing codes are standard. The frequency is unremarkable. None of the individual data points triggers a flag. But the sequence of the authorization request, the documentation submitted, the approval pattern, and the subsequent billing follows a structure that doesn’t appear in legitimate claims, because no legitimate provider needs to structure their documentation that way.
Behavioral sequence analysis requires a different kind of attention than statistical modeling. It requires someone who understands what the legitimate process looks like, including which shortcuts and variations are normal versus which combinations are only coherent as fraud infrastructure.
The Documentation Break: Where Fraud Actually Lives
Fraud doesn’t just produce financial anomalies. It produces documentation anomalies: records that were created to satisfy requirements rather than to reflect reality. These are different from records that were created carelessly. They’re different in specific, consistent ways.
Documentation breaks in fraudulent claims have a signature. Certain fields that should vary across legitimate claims are suspiciously consistent. Supporting documentation arrives too quickly after a request, because it was prepared in advance. Dates cluster in ways that reflect administrative convenience rather than clinical reality. Authorization documentation contains language that tracks too closely to the requirement standard, because it was written to match the standard rather than to describe an actual interaction.
These breaks are invisible to automated detection because they’re not statistical anomalies. They’re procedural anomalies. They require human pattern recognition trained on what legitimate records look like in context. A model that has never been shown what a correctly documented authorization looks like cannot flag one that was constructed backward.
The Stalled Case: Where This Gets Expensive
The cost of this detection gap doesn’t show up immediately. It shows up in your stalled case queue.
Most SIU teams have a category of cases that opened on reasonable suspicion, were worked by investigators, and never moved to resolution: not because the case turned out clean, but because the investigation couldn’t build a case that held up. The suspicion was valid. The record review didn’t surface the evidence needed to move forward.
These cases represent a specific problem. The fraud is real enough to trigger an investigation, but the detection methodology being applied isn’t designed to surface the evidence that actually proves the scheme. The case stays open. The scheme continues to run. At some point the case closes on insufficient evidence, and the scheme, now confident it survived review, typically escalates.
In my experience reviewing stalled SIU cases, the most common finding isn’t that the case was unfounded. It’s that the investigation was looking at the wrong layer of the record: focusing on billing outputs rather than documentation sequence, or on individual claim analysis rather than provider-level behavioral pattern.
What Catches What Your Model Doesn’t
The gaps in automated detection are filled by a different kind of analysis: one that examines the record forensically rather than statistically. Four methods do most of the work.
Documentation sequence analysis maps the order and timing of records created across a claim or case, looking for patterns inconsistent with legitimate process flow. The flags are records prepared before the event they document, supporting records with internal timestamps inconsistent with submission dates, and approval records that post-date the actions they authorized.
Cross-record integrity review examines whether records created by different parties at different times tell a consistent story. The flags are medical records that don’t match authorization requests that don’t match billing submissions, communications referencing events before they allegedly occurred, and records sharing language with implausible precision.
Procedural gap identification looks for steps in a required process that are absent from the record entirely. The flags are authorization processes missing required clinical notes, required verification steps documented as completed without supporting evidence, and escalation requirements not reflected in the record despite trigger conditions being present.
Behavioral pattern mapping identifies the specific sequence of provider actions that characterizes a scheme rather than individual anomalous data points. The flags are providers whose documentation patterns change after an audit cycle in ways that track specifically to the audit findings, and authorization patterns that cluster around billing thresholds in ways inconsistent with clinical distribution.
The Honest Assessment of Your Current Program
No detection program catches everything. That’s not a failure of the program. It’s the nature of the problem. Fraud adapts. Detection follows.
The question isn’t whether your program has gaps. It does. Every program does. The question is whether you know where the gaps are, and whether the cases currently sitting in your stalled queue are there because of those specific gaps.
If your team has cases that have been open for six months or more with no clear path to resolution, those are worth a forensic second look. Not because the original investigation was wrong, but because a different analytical lens may surface the evidence your detection model wasn’t designed to find.
